Health Coordinator -  Fixed Term 9 months 



Millstream House is an established home which offers 24 hour residential care. Our residents are living with Huntington’s disease and other conditions such as ABI and physical disabilities. We also offer respite support.

The Health Coordinator in conjunction with, and under the instruction of the home / service manager, will assist with the day-to-day supervision and management of the service.

The key focus of this role is on the health care requirements of the people we support including; management and oversight of the medication systems, implementation and maintaining of health related support guidance and record keeping, ensuring all people we support receive appropriate health screening, referrals and appointments.

Contributing to, and being responsible for elements of, the assessment, planning and implementation of, in partnership with the people you support, programmes of support and / or care.


Our ideal candidate will have the ability to multi-task as they will be working in a busy fast paced office environment. They will need to be able to work unsupervised and must be capable of dealing with clients and their families, care team staff  and other healthcare professionals.

Due to the nature if this role it is essential you have the following:

  • Excellent verbal/written communication skills
  • In the absence of the Service Manager, the ability to lead, motivate and manage a  large staff team
  • Administrative skills
  • Knowledge of relevant regulations and legislation

Desirable you hold a NVQ 3 in Care Management and essential you are willing to work towards NVQ 4 in Care and Management

  • Experience of multidisciplinary approach to service delivery.
  • Experience within a Huntington’s Disease care provision.
  • Experience of working with key professionals e.g. SALT, Occupational Therapy, GPs, Medical Suppliers

Knowledge of relevant healthcare regulations and legislation
