Focused Healthcare Specialist Children’s Complex Healthcare providers are a team of Registered Clinical Nurse Managers, Clinical Educators, Paediatric Nurses and Healthcare Assistants that provide health care support to children and young people within their own home and educational settings.

Role Overview

The Clinical Nurse Manager will be motivated and innovative, with strong leadership, management and communication skills, to ensure that the care delivered is safe, effective and reliable. Working with children and young people with complex healthcare needs (and their families), they will assess, plan, implement and evaluate evidence-based, family centred care and collaborate with professionals across agencies and sectors ensuring effective service delivery.

The Clinical Nurse Manager will also assist, as part of the wider team, in the development and delivery of our complex care services. This will require being responsive to need and to communication through co production of packages.

 

Key Responsibilities

 

  • Accountable for management of a clinical case load in the community setting;
  • To organise and prioritise workload;
  • Be effective with your own time management and colleague’s time management, by having a responsive approach when completing visits. This may include supporting Clinical Educators role whilst completing visits to families’ homes;
  • To provide back up and “Buddy” support for your clinical team colleagues;
  • Carry out staff appraisals and supervisions for Nursing staff and Healthcare Assistants;
  • To assess, plan, implement and regularly evaluate the child/ young person’s holistic care, in partnership with the family and multidisciplinary team, ensuring documentation is maintained and updated;
  • To maintain accurate and contemporaneous records, in line with local policies and NMC guidelines and ensuring up to date care plans are in place;
  • To undertake clinical audits and be involved in developing and leading service improvements;
  • To conform to NMC requirements, by keeping up to date with current trends and developments in nursing and demonstrate evidence of such;
  • To ensure nursing practice is in accordance with the NMC Codes of Conducts;
  • Maintaining a flexible working pattern to meet the Clinical needs of the care package for the child/ young person;
  • To participate in clinical governance and risk management procedures, in line with policies and guidelines;
  • To ensure clinical visits occur during reasonable working hours, in line with the child/ young person’s needs and family preferences;
  • To be aware of the Focused Healthcare Complaints Policy, and be able to take appropriate action, implement and undertake investigations as per process;
  • Signpost families to appropriate external resources;
  • Liaise and communicate effectively with all external professionals involved with the child/ young person and family, including working in partnership with the child’s social worker and Continuing Care Nurse;
  • Attend all TAC (Team Around the Child), LAC (Looked After Child) and MDT(Multidisciplinary Team) meetings and child protection conferences and strategy meetings as required;
  • Provide advocacy and a voice for the child/ young person and family;
  • Provide detailed reports for conferences and quarterly meetings;
  • Provide information to enable case studies to be written on request;
  • Engage in dialogue with Bookings Team, offering knowledge to enable them to find solutions, working collaboratively;
  • Share information with professional colleagues where it is in the best interest of the child/ young person and family;
