Care Coordinator

Location: Strood

Millstream House is an established home which offers 24 hour residential care. Our residents are living with Huntington’s disease and other conditions such as ABI and physical disabilities. We also offer respite support.

The Care Coordinator will report to the management team and be responsible for the coordination of the care delivery team, to ensure provision of commissioned services to the people we support. They will be the first point of contact for day to day requirements of the care team as well as the people we support.

 

Our ideal candidate will have the ability to multi-task as they will be working in a busy fast paced office environment. They will need to be able to work unsupervised and must be capable of dealing with clients and their families, care team staff  and other healthcare professionals.

You must have experience working in a healthcare environment as a coordinator or similar role and be prepared to participate in our on call service on a rota basis.

Desirable you hold a NVQ 3 in Care Management and essential you are willing to work towards NVQ 4 in Care and Management.

You must have at least 3 years supervisory experience in a care setting covering the following:

Excellent verbal/written communication skills

Managing large team rotas

Engaging in active recruitment

Induction training

Budgetary/financial and administrative skills

Understanding Care/Support Plans with a clinical focus - supporting to keep these updated

Knowledge of relevant healthcare regulations and legislation
